One form for all K-8 projects
Above all else, we want students to perform experiments safely and humanely. Teachers are encouraged to talk to their students about their project safety before experimentation begins. Once you've had this discussion about safety, students will have a better understanding of their projects and the procedures they need to design to conduct their experiments.
A Scientific Review Board at your school is tasked with reviewing the project contents, steps, and procedures to ensure students are avoiding harmful or dangerous studies, that humans and animals are being treated humanely, that test subjects are not being put at risk, and verifying that the student is taking the necessary precautions to perform the experiment safely.
An SRC at your school would consist of a principal/administrator, teacher (other than the one overseeing the project), and a counselor or nurse. If the experiment uses animals, an additional signature of a veterinarian is required.
Fill in the form below, before experimentation begins.
Send the completed form to us at YESfair@ssvec.com
This should be done for each project you are sending to YES Fair.
